Please send all documents via fax or email.
HOUSE WATCH forms WILL be accepted via email only and can be emailed to smangum@piedmont-ok.gov
SPECIAL EVENTS forms must be submitted to the Police department at least two (2) weeks prior to the event. This can be submitted to dsterling@piedmont-ok.gov

Requesting Police Records
To receive police reports or records from the Piedmont Police Department, please follow the steps below. Payment must be received before records can be released.
Step 1: Submit a Records Request
Visit https://www.piedmont-ok.gov/220/Police-Documents-Record-Request and complete the online Records Request Form.
Step 2: Pay the Required Fee
• $2.00 per report
• $20.00+ for body camera footage (cost may vary based on length)
• Note: We do not use dash cameras.
Step 3: 911 Recordings
To request a 911 call recording, a subpoena is required. Please email the subpoena to both:
• ypdrecords@yukonok.gov
• dsterling@piedmont-ok.gov
These recordings are handled by our PSAP 911 Station.
Step 4: Mail Payment and Forms
Send the following to:

Include:
• Payment for the requested records
• A completed Records Release Form
• A copy of your driver’s license (for identity verification)
• Your email address if you prefer to receive the records electronically
